摘要/Abstract

摘要： 目的评估乌鲁木齐市HIV阴性男男性行为者（MSM）肛门HPV 6、16型反复感染的特征及影响因素。方法 2016—2021年与乌鲁木齐市非政府组织合作招募MSM,每6个月随访1次,收集MSM肛周脱落细胞进行HPV基因分型,分析HPV 6、16型的年龄分布特征,采用Logistic回归分析探讨反复感染的影响因素。结果本研究纳入626名MSM,年龄30（25,36）岁,随访时间36.63（22.18,52.10）月,随访次数5（3,7）次。HPV 6、16型基线感染率分别为10.26%（95%CI：8.23%~12.64%）、9.09%（95%CI：7.19%~11.35%）。37种型别中反复感染率最高的是HPV 6型和16型,分别为13.9%（95%CI：11.13%~17.14%）和9.74%（95%CI：7.45%~12.52%）,且呈现年龄特异性,<24岁MSM HPV 6型反复感染率高于其他年龄组,而HPV 16反复感染率则随年龄增加而升高。调整人口学和性行为特征后,同性肛交性行为增加HPV 6型反复感染机率（αOR=1.637）。结论 HPV 6、16型反复感染率较高,有必要在MSM尤其是年轻MSM中接种HPV疫苗,预防HPV相关疾病。

Abstract: Objective To evaluate the characteristics and influencing factors of recurrent anal infection of HPV 6 and 16 in HIV-negative men who have sex with men （MSM） in Urumqi. Methods From 2016 to 2021, MSM were recruited in cooperation with nongovernmental organizations in Urumqi, followed up once every 6 months, collected perianal explants for HPV genotyping, analyzed the age distribution characteristics of HPV 6 and HPV 16, and Logistic regression analysis was used to explore the influencing factors of recurrent infection. Results A total of 626 MSM were enrolled in this study, with a median age of 30 （25, 36） years, a median follow-up time of 36.63 （22.18, 52.10） months and 5 （3, 7） follow-up times. The baseline infection rates of HPV 6 and 16 were 10.26% （95%CI： 8.23%-12.64%） and 9.09% （95%CI： 7.19%-11.35%）, respectively. Among the 37 types, the highest recurrent infection rates were HPV 6 and 16 [13.9% （95%CI： 11.13%-17.14%） and 9.74% （95%CI： 7.45%-12.52%）, respectively], and showed age-specific. The recurrent infection rate of HPV 6 of <24 years old MSM was higher than that of other age groups, while the recurrent infection rate of HPV 16 increased with age. After adjusting for demographic and sexual behavior characteristics, the risk of recurrent HPV 6 infection was increased in men who had anal sex with men （αOR=1.637）. Conclusions The recurrent infection rate of HPV 6 and 16 is high. It is necessary to vaccinate the HPV vaccine in MSM, especially young MSM, to prevent HPV-related diseases.
