安徽地区高血压患者健康相关行为改善现状及其对血压控制达标影响因素研究

Study on the improvement of health-related behaviors and its influence on blood pressure control in hypertensive patients in Anhui

摘要: 目的 调查高血压患者健康相关行为改善现状并分析其对血压控制达标影响因素。方法 选取2021年1月至2023年3月在安徽地区4家医院治疗的高血压患者作为研究对象,通过健康行为改善得分评估患者健康相关行为改善情况,收集患者性别、年龄、婚姻状况、文化程度、膳食情况、吸烟史、饮酒史、体育锻炼、按医嘱服药、心理状况等基本资料,并对其血压控制情况进行调查,采用描述性分析方法对该人群健康相关行为改善情况及血压控制达标情况进行分析,采用单、多因素分析方法分析健康相关行为对高血压患者血压达标的影响因素。结果 本研究共纳入有效样本1 986例,男1 265例,女721例,年龄19~81岁,病程1~17年。高血压患者的健康行为改善得分为(2.37±0.52)分。多因素logistic回归分析结果显示,吸烟(OR=0.142)、不限制饮酒(OR=0.126)、规律运动(OR=9.052)、合理膳食(OR=1.328)、按医嘱服药(OR=1.283)、保持健康体重(OR=1.317)是高血压患者血压控制的影响因素。结论 在对高血压患者进行健康管理后其健康相关行为改善情况良好,吸烟、饮酒、规律运动、合理膳食、按医嘱服药、保持健康体重等健康相关行为是患者血压控制达标的影响因素。

Abstract: Objective To investigate the status of health-related behavior improvement in hypertensive patients and analyze its influencing factors on blood pressure control. Methods Selecting hypertensive patients who were treated in four hospitals in Anhui Province from January 2021 to March 2023 for this study, the improvement of health-related behaviors was evaluated through health behavior improvement scores. Basic information such as gender, age, marital status, education level, dietary status, smoking history, drinking history, physical exercise, taking medicine as directed, psychological status, etc. were collected, and their blood pressure control status was investigated. Descriptive analysis was used to analyze the improvement of health-related behaviors and the achievement of blood pressure control in this population. Univariate and multivariate analyses were used to analyze the influence of health-related behaviors on blood pressure control in hypertensive patients. Results A total of 1 986 valid samples were included in this study, including 1 265 males and 721 females, aged 19-81 years, with a disease course of 1-17 years. The score of health-related behavior improvement in hypertensive patients was (2.37±0.52) points. Multivariate logistic regression analysis showed that smoking (OR=0.142), no restriction on drinking (OR=0.126), regular exercise (OR=9.052), reasonable diet (OR=1.328), taking medicine as directed (OR=1.283), and maintaining a healthy weight (OR=1.317) were the influencing factors of blood pressure control in hypertensive patients. Conclusions After implementing health management, hypertensive patients demonstrated significant improvements in their health-related behaviors. Smoking, drinking, regular exercise, reasonable diet, taking medicine as directed, and maintaining a healthy weight are the influencing factors of blood pressure control.
