医护人员排班模式与职业性损伤的关联——基于多水平模型的科室间差异分析

Association between shift scheduling patterns and occupational injuries among healthcare workers: A multilevel analysis of inter-departmental variations

摘要: 目的 探讨医护人员排班模式特征与职业性损伤的关联性,分析科室层面因素对上述关联的修饰效应。方法 采用多中心横断面调查设计,于2025年5月选取西安市3家三甲医院630名医护人员为研究对象,采用调查问卷收集排班情况与职业性损伤数据,构建两水平logistic回归模型(个体为水平1,科室为水平2)分析排班模式与职业性损伤的关联。结果 回收有效问卷600份,有效回收率95.24%。职业性损伤总体发生率82.17%(493/600),其中内科78.02%(181/232)、外科80.93%(157/194)、急诊科88.89%(96/108)、ICU 89.39%(59/66)。内科以常白班为主,外科以常白班与两班倒为主,急诊科与ICU以两班倒与三班倒为主。两水平logistic回归显示,控制混杂因素后,外科(OR=1.972)、急诊科(OR=2.254)、ICU(OR=2.375)损伤风险均高于内科(均P<0.05);床护比<1∶0.4(OR=4.873,P<0.05)亦与损伤风险升高相关。与常白班相比,两班倒(OR=2.023)、三班倒(OR=1.988)、夜班≥9次/月(OR=2.777)、周均工时>40 h(OR=2.389)及每周加班≥2次(OR=2.607)均为职业性损伤的独立关联因素(均P<0.05)。纳入固定效应后科室随机截距残留方差降至0.208(P=0.035)。结论 医护人员职业性损伤存在明显的个体差异与科室聚集性,不合理排班模式与职业性损伤风险升高呈显著关联,且关联强度受科室类别影响。

Abstract: Objective To investigate the association between shift scheduling patterns and occupational injuries among healthcare professionals, and to analyze the moderating effects of department-level factors on this association. Methods A multicenter cross-sectional study design was employed. In May 2025, 630 healthcare professionals from three tertiary Grade-A hospitals in Xi'an were recruited as the study population. Data were collected via questionnaire surveys. A two-level logistic regression model (with individuals as Level 1 and departments as Level 2) was constructed to analyze the correlation between shift scheduling patterns and the incidence of occupational injuries. Results A total of 600 valid questionnaires were retrieved, yielding an effective response rate of 95.24%. The overall prevalence of occupational injuries was 82.17% (493/600), with distribution rates of 78.02% (181/232) in internal medicine, 80.93% (157/194) in surgery, 88.89% (96/108) in the emergency department (ED), and 89.39% (59/66) in the intensive care unit (ICU). Internal medicine predominantly utilized regular day shifts; surgery primarily employed regular day and two-shift systems; whereas the ED and ICU mainly implemented two-shift and three-shift systems. Multilevel logistic regression analysis revealed that, after controlling for confounding variables, the risk of occupational injury was significantly higher in surgery (OR=1.972), the ED (OR=2.254), and the ICU (OR=2.375) compared to internal medicine (P<0.05). A bed-to-nurse ratio of <1∶0.4 (OR=4.873, P<0.05) was also associated with an elevated injury risk. Furthermore, compared to regular day shifts, the two-shift system (OR=2.023), three-shift system (OR=1.988), ≥9 night shifts per month (OR=2.777), average weekly working hours >40 h (OR=2.389), and overtime frequency of ≥2 times per week (OR=2.607) emerged as independent correlates of occupational injuries (all P<0.05). Following the inclusion of fixed effects, the residual variance of the random intercept at the department level decreased to 0.208 (P=0.035). Conclusion Occupational injuries among healthcare professionals exhibit pronounced individual heterogeneity and departmental clustering. Suboptimal shift scheduling patterns are significantly associated with an elevated risk of occupational injury, and the magnitude of this association is moderated by department typology.
