云南省中学生意外伤害与心理健康症状相关关系研究

The relationship between unintentional injury and mental health symptoms among middle school students in Yunnan Province

摘要: 目的 探究云南省中学生意外伤害发生及与心理健康症状的相关关系,为制定有针对性的预防伤害健康教育措施,推动和促进儿童青少年健康提供科学依据。方法 于2023年10—11月采用多阶段分层随机整群抽样方法,选取云南省4州(市)8县(区)4 172名12~18岁在校中学生为研究对象。应用自行编制的调查问卷收集学生的一般人口学特征、伤害发生相关情况等,采用流调中心标准的抑郁量表(CES-D)评估心理健康症状。采用Spearman相关、多因素logistic回归等方法分析心理健康症状与意外伤害发生的相关性。结果 抑郁情绪维度得分(rs=0.112)、躯体症状维度得分(rs=0.119)、人际关系维度得分(rs=0.088)与意外伤害的发生呈正相关,积极情绪维度得分(rs=-0.043)与意外伤害的发生呈负相关(均P<0.01)。经多因素logistic回归分析显示,控制性别、学段后,躯体症状程度维度得分与意外伤害的发生有关(OR=1.060)。进一步控制地区、家庭等因素并将心理症状按平均水平划分后发现,抑郁情绪(OR=1.394)与躯体症状(OR=1.701)处于高水平与中学生意外伤害发生相关(均P<0.05)。结论 中学生意外伤害的发生与其特定的心理健康症状密切相关,当中学生抑郁情绪与躯体症状达到较高水平时,意外伤害风险显著增加。应针对伤害发生的危险因素采取综合的干预措施,构建多元支持系统,提升青少年自我调适能力,以降低意外伤害发生的风险。

Abstract: Objective To investigate the incidence of unintentional injuries and its correlation with mental health symptoms among middle school students in Yunnan Province, so as to provide a scientific basis for the formulation of targeted health education measures for injury prevention and for the promotion of child and adolescent health. Methods A multi-stage stratified random cluster sampling method was employed from October to November 2023 to select 4 172 middle school students aged 12-18 from eight counties (districts) in four prefectures (cities) of Yunnan Province as research subjects. A self-compiled questionnaire was used to collect data on students' general demographic characteristics and injury-related information. The Center for Epidemiologic Studies Depression Scale (CES-D) was utilized to assess mental health symptoms. Spearman correlation and multivariate logistic regression were used to analyze the correlation between mental health symptoms and the occurrence of unintentional injuries. Results Scores on the dimensions of depressive affect (rs=0.112), somatic symptoms (rs=0.119), and interpersonal relations (rs=0.088) were positively correlated with the occurrence of unintentional injuries, whereas the positive affect dimension score (rs=-0.043) was negatively correlated (all P<0.01). After controlling for gender and academic stage, multivariate logistic regression analysis indicated that the somatic symptom dimension score was associated with the occurrence of unintentional injuries (OR=1.060). After further controlling for factors such as region and family, and categorizing psychological symptoms by their mean levels, it was found that high levels of depressive affect (OR=1.394) and somatic symptoms (OR=1.701) were correlated with the occurrence of unintentional injuries among middle school students. Conclusion The occurrence of unintentional injuries in middle school students is closely associated with specific mental health symptoms. The risk of unintentional injury increases significantly when students' depressive affect and somatic symptoms reach high levels. Comprehensive intervention measures should be implemented targeting the risk factors for injuries, a multi-tiered support system should be constructed, and adolescents' self-regulation abilities should be enhanced to reduce the risk of unintentional injury.
