2型糖尿病合并其他慢性病患者服药依从性调查

Investigation of medication compliance in patients with type 2 diabetes mellitus combined with other chronic diseases

摘要: 目的 调查分析2型糖尿病合并其他慢性病患者的服药依从性,并探讨患者服药依从性可能的影响因素。方法 采用方便抽样法抽取2021年6月至2023年6月于东部战区总医院、南京市浦口区桥林街道社区卫生服务中心、南京市江东社区卫生服务中心就诊的2型糖尿病患者,调查患者的服药依从性及合并其他慢性病情况,经单因素、多因素分析2型糖尿病合并其他慢性病患者服药依从性的影响因素。结果 本研究共有效调查1 135例2型糖尿病患者,其中服药依从性差者453例,占39.91%;1 135例患者中874例为2型糖尿病合并其他慢性病,2型糖尿病合并其他慢性病患者的服药依从性差占比为44.97%(393/874)。多因素logistic回归分析结果显示初中及以下受教育程度(OR=5.749)、合并心脑血管疾病或慢性呼吸系统疾病(OR=2.482、3.313)、服药种类≥3种(OR=3.377)、用药知识掌握情况(OR=0.391、0.328)、过去6个月未接受过电话或面对面随访(OR=7.128)均可能是2型糖尿病合并其他慢性病患者服药依从性差的影响因素。结论 2型糖尿病合并其他慢性病患者的总体服药依从性较差,患者服药依从性有待提高,应针对影响该患者人群服药依从性的相关因素继续开展疾病知识普及、用药知识宣教、简化药物治疗等干预措施的改善工作。

Abstract: Objective To investigate and analyze the medication compliance of patients with type 2 diabetes mellitus combined with other chronic diseases, and explore the possible influencing factors of medication compliance. Methods Patients with type 2 diabetes mellitus who were admitted to the General Hospital of Eastern Theater Command, Community Health Service Center of Qiaolin Street, Pukou District, Nanjing City, and Nanjing Jiangdong Community Health Service Center from June 2021 to June 2023 were selected by convenient sampling method to investigate their medication compliance and complications of other chronic diseases. The influencing factors of medication compliance in patients with type 2 diabetes mellitus combined with other chronic diseases were analyzed by univariate and multivariate analysis methods. Results A total of 1 135 patients with type 2 diabetes mellitus were investigated in this study, among which 453 patients (39.91%) had poor medication compliance. Among the 1 135 patients, 874 were combined with other chronic diseases, and the proportion of poor medication compliance was 44.97% (393/874). Multivariate logistic regression analysis showed that education level of junior high school or below (OR=5.749), combined with cardiovascular and cerebrovascular diseases or chronic respiratory diseases (OR=2.482, 3.313), types of medication ≥3 (OR=3.377), knowledge of medication (OR=0.391, 0.328), and without phone or face-to-face follow-up within the past 6 months (OR=7.128) were influencing factors for poor medication compliance in patients with type 2 diabetes mellitus combined with other chronic diseases. Conclusions The overall medication compliance of patients with type 2 diabetes mellitus combined with other chronic diseases is poor and needs to be improved. The improvement of intervention measures such as disease knowledge popularization, medication knowledge education, and simplified drug therapy should be carried out according to the relevant factors affecting the medication compliance of this population.
