自然人群代谢异常聚集与颈动脉内膜中层厚度关联性分析

Association between the clustering of metabolic abnormalities and carotid intima-media thickness in the general population

摘要: 目的 基于35~75岁自然人群,探讨颈动脉内膜中层厚度(cIMT)与多代谢异常聚集的相关性。方法 采用整群随机抽样方法,于2022年5月至2025年4月选取嘉兴市5县2区的938名常住居民为研究对象,调查一般人口学资料,进行体格检查及生化指标检测,记录代谢异常指标数量;测量并记录cIMT值。以代谢异常数量为自变量、cIMT为因变量进行线性回归分析,并进一步采用多因素logistic回归分析(以cIMT增厚为因变量),校正年龄、性别等混杂因素后分析多代谢异常聚集对cIMT增厚的影响。结果 938名35~75岁自然人群中,代谢异常指标数量0项339人(36.14%),1项276例(29.42%),2项186例(19.83%),≥3项137例(14.61%)。随着代谢异常指标数量增多,cIMT逐渐增加(P<0.05)。多元线性回归分析显示,代谢异常数量每增加1个,cIMT平均升高0.050 mm(95% CI:0.042~0.058)。多因素logistic回归分析显示,年龄60~75岁(OR=1.443)、男性(OR=1.468)、当前吸烟(OR=1.414)、大量饮酒(OR=1.682)、体力活动不达标(OR=1.348)、BMI≥28.0 kg/m²(OR=1.504)、腰臀比(每增加0.1,OR=1.138)及代谢异常指标数量(2项:OR=1.685;≥3项:OR=1.845)均为cIMT增厚的相关因素(均P<0.05)。结论 在自然人群中,多代谢异常指标聚集与cIMT升高呈正相关,随着个体代谢异常指标数量增加,cIMT增厚的发生风险逐渐升高。

Abstract: Objective To investigate the association between carotid intima-media thickness (cIMT) and the clustering of multiple metabolic abnormalities within a general population age 35-75 years. Methods Employing a cluster random sampling method, 938 permanent residents from five counties and two districts of Jiaxing City were enrolled as the study cohort between May 2022 and April 2025. General demographic data were collected, physical examinations and biochemical assessments were conducted, and the number of metabolic abnormality indices was recorded. cIMT values were quantitatively measured and documented. A linear regression analysis was performed utilizing the number of metabolic abnormalities as the independent variable and cIMT as the dependent variable. Furthermore, a multivariate logistic regression analysis (with cIMT thickening as the dependent variable) was conducted to evaluate the impact of the clustering of multiple metabolic abnormalities on cIMT thickening, following adjustments for confounding variables including age and sex. Results Among the 938 participants age 35-75 years from the general population, the prevalence of metabolic abnormality indices was as follows: 0 abnormalities in 339 individuals (36.14%), 1 in 276 individuals (29.42%), 2 in 186 individuals (19.83%), and ≥3 in 137 individuals (14.61%). A statistically significant, progressive increase in cIMT was observed concurrently with an escalation in the number of metabolic abnormality indices (P<0.05). Multiple linear regression analysis revealed that each single-unit increase in the number of metabolic abnormalities was associated with a mean cIMT elevation of 0.050 mm (95% CI: 0.042-0.058). Multivariate logistic regression analysis delineated that age 60-75 years (OR=1.443), male sex (OR=1.468), current smoking (OR=1.414), heavy alcohol consumption (OR=1.682), insufficient physical activity (OR=1.348), BMI≥28.0 kg/m² (OR=1.504), waist-to-hip ratio (OR=1.138 per 0.1 increment), and the number of metabolic abnormalities (2 abnormalities: OR=1.685; ≥3 abnormalities: OR=1.845) were all significantly associated factors for cIMT thickening (all P<0.05). Conclusion In the general population, the clustering of multiple metabolic abnormalities exhibits a positive correlation with elevated cIMT. The risk of developing cIMT thickening escalates progressively in tandem with the increasing number of metabolic abnormalities within an individual.
