2015—2024年广东省东莞市人间布鲁氏菌病流行特征及时空聚集分析

Epidemiological characteristics and spatiotemporal cluster analysis of human brucellosis in Dongguan, 2015-2024

摘要: 目的 分析2015—2024年东莞市人间布鲁氏菌病(布病)的流行特征和时空聚集性,为布病防控措施制定和调整提供科学依据。方法 收集2015—2024年东莞市人间布病报告病例相关数据,描述病例流行特征,采用Joinpoint回归分析人间布病发病趋势,采用Arc-Map 10.8.2软件进行全局空间自相关和热点分析,通过SaTScan 10.1软件进行时空聚集性分析。结果 2015—2024年东莞市共报告人间布病病例221例,年报告发病率无明显变化(AAPC=1.54%,P=0.624),月报告发病率有2个上升时期,分别是2015年1月到2016年4月(MPC=8.10%,P=0.037)和2024年2—5月(MPC=70.44%,P=0.004)。人间布病发病主要集中在3—8月,占全年总病例数的72.85%(161/221)。发病男女性别比为2.88∶1,职业人群和非职业人群最高暴露因素均是接触羊及其制品。2018—2024年血清学累计监测职业人群1 672人,阳性率为0.54%(9/1 672)。全局空间自相关分析显示除2018年外,其余各年度病例分布为随机排列。热点分析显示出现次数最多的为桥头镇。时空扫描显示,2015—2024年一类聚集区域在桥头镇和谢岗镇,二类聚集区域在石龙镇和茶山镇。结论 东莞市人间布病发病总体平稳,但在部分地区呈现明显的时空聚集性,需加强热点区域和时空聚集区域的布病防控。

Abstract: Objective To analyze the epidemiological characteristics and spatiotemporal clustering of human brucellosis in Dongguan City from 2015 to 2024, thereby providing a scientific basis for the formulation and refinement of prevention and control strategies. Methods Data pertaining to reported cases of human brucellosis in Dongguan City from 2015 to 2024 were collected. The epidemiological characteristics were described using descriptive statistics. Joinpoint regression analysis was employed to assess incidence trends. Global spatial autocorrelation and hotspot analyses were conducted using ArcMap 10.8.2, while spatiotemporal clustering analysis was performed with SaTScan 10.1. Results A total of 221 human brucellosis cases were reported in Dongguan City from 2015 to 2024. The annual reported incidence exhibited no significant trend (AAPC=1.54%, P=0.624). However, the monthly reported incidence revealed two periods of significant increase: January 2015 to April 2016 (MPC=8.10%, P=0.037) and February to May 2024 (MPC=70.44%, P=0.004). Case incidence was concentrated between March and August, accounting for 72.85% (161/221) of the total annual cases. The male-to-female ratio was 2.88∶1. The principal exposure risk factor for both occupational and non-occupational populations was contact with sheep/goats and their products. Serological surveillance of 1 672 occupationally exposed individuals from 2018 to 2024 revealed a positivity rate of 0.54% (9/1 672). Global spatial autocorrelation analysis indicated a random distribution of cases for all years except 2018. Hotspot analysis identified Qiaotou Town as the most frequent hotspot. Spatiotemporal scan statistics detected a primary cluster in Qiaotou and Xiegang towns and a secondary cluster in Shilong and Chashan towns over the 2015-2024 period. Conclusion The overall incidence of human brucellosis in Dongguan City is relatively stable; however, the disease demonstrates significant spatiotemporal clustering in specific locales. Enhanced prevention and control measures should be prioritized in these identified hotspot and clustered areas.
