云南民族地区学龄儿童青少年意外伤害的流行现况及相关影响因素

Prevalence and associated factors of unintentional injuries among school-aged children and adolescents in ethnic minority regions of Yunnan Province

摘要: 目的 分析云南省多民族地区学龄儿童青少年的意外伤害流行特征及其相关影响因素,为制定有效的预防与控制措施提供科学依据。方法 采取分层随机整群抽样的方法,以云南省少数民族地区8~16岁学生为研究对象,采用自行设计的问卷开展意外伤害流行病学调查。采用非条件二元logistic回归模型分析发生意外伤害的影响因素。结果 共招募云南省少数民族地区学龄儿童青少年8~16岁学生2 627名,自我报告的意外伤害发生率为71.22%(1 871例),男女比例为1∶0.92。过去1年中,发生过打架行为者(OR=1.71),经常自觉学业压力大者(OR=1.55),过去1年有持续感到悲伤和绝望者(OR=1.30)、经常自觉有孤独感(OR=1.53)以及母亲从事非农业工作(OR=1.27)和留守儿童(OR=1.21)是云南民族地区学龄儿童青少年发生意外伤害风险的独立危险因素(均P<0.05)。结论 云南省民族地区学龄儿童青少年的意外伤害问题不容忽视。建议在开展儿童青少年意外伤害健康教育时,协同增加儿童青少年心理健康干预,从而有效的降低儿童青少年意外伤害的发生风险和疾病负担。

Abstract: Objective To investigate the epidemiological characteristics and associated risk factors of unintentional injuries among school-aged children and adolescents in the multi-ethnic regions of Yunnan Province, in order to provide a scientific basis for the formulation of effective prevention and control strategies. Methods A stratified random cluster sampling method was employed to conduct an epidemiological survey on unintentional injuries. The study population comprised students aged 8 to 16 years from ethnic minority areas in Yunnan Province, who were administered a self-designed questionnaire. Unconditional binary logistic regression analysis was utilized to identify the determinants of unintentional injuries. Results A total of 2 627 school-aged children and adolescents were recruited from the ethnic minority regions of Yunnan Province. The self-reported incidence rate of unintentional injuries was 71.22% (1 871 cases), with a male-to-female ratio of 1∶0.92. Students who had engaged in physical fighting in the past year (OR=1.71), those who frequently perceived high academic pressure (OR=1.55), those who had experienced persistent feelings of sadness and hopelessness in the past year (OR=1.30), those who frequently felt lonely (OR=1.53), as well as those whose mothers were engaged in non-agricultural occupations (OR=1.27) and left-behind children (OR=1.21) were identified as independent risk factors for unintentional injury among school-aged children and adolescents in ethnic minority areas of Yunnan Province (P<0.05). Conclusion The problem of unintentional injury among school-aged children and adolescents in multi-ethnic regions require urgent attention. The prevention and control of unintentional injuries among children in ethnic areas should recommend to implement health education on unintentional injury prevention, psychological health interventions be concurrently integrated, thereby effectively reducing the risk and burden of unintentional injuries among children and adolescents in ethnic areas.
